We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

If you provide us with complete insurance information, we will be happy to submit a claim if applicable. Every effort will be made to closely estimate your co-payment and deductible, but the ultimate responsibility for any unpaid balances is yours. Please understand that insurance is a contract between you and your insurance carrier. If you have any questions regarding your insurance, we will do our best to help you, but we ask that you contact your company regarding the specifics and details of the plan it is contracted with on your behalf.
